SPRINGFIELD, Pa. – The Crozer-Keystone Center for Geriatric Medicine, led by William Zirker, M.D., chief of Geriatrics, has consolidated its outpatient offices in Crum Lynne, Upland and Media into one central location in Springfield. The practice will share a suite at Springfield Hospital with the Crozer-Keystone Senior Health Services department (100 W. Sproul Road, Healthplex Pavilion II, Suite 221). The move provides a central location where aging adults, their family members and caregivers can access Crozer-Keystone’s comprehensive community referral and geriatric medical services.

The Center for Geriatric Medicine offers specialized care provided by a clinical team of board-certified geriatricians, a physician’s assistant and a social worker. The practice provides a full range of consultative and primary care services for patients over the age of 50. This includes the comprehensive Geriatric Evaluation and Management Program, which is an intensive evaluation for medical, cognitive, functional and medication problems that can interfere with daily activities; behavioral issues and much more. Results form the basis of a care plan that is shared and implemented with the patient’s primary care physician and the geriatric team.

The Center’s geriatricians also care for patients in nursing homes throughout Delaware County and serve as medical directors for three – including The Belvedere in Chester, Wallingford Nursing and Rehabilitation Center in Wallingford, and Sterling Healthcare & Rehabilitation Center in Media.

The Crozer-Keystone Senior Health Services department has been a trusted community resource for more than 15 years. Its Senior Support Line, 1-800-CKHS-KEY (1-800-254-7539), is a direct connection to programs and services to help aging adults manage health and lifestyle changes.
